Understanding Prolapsed Hemorrhoids: Grades, Symptoms, and Treatment Options

If you’ve noticed a small bulge during a bowel movement, light bleeding, or a feeling of pressure that doesn’t quite go away, you may be dealing with prolapsed hemorrhoids. It’s a topic many people hesitate to talk about, but you should know it’s common and very treatable.

“Patients are often embarrassed to bring up hemorrhoid symptoms,” says Desiree D’Angelo, DO, FACS, board-certified surgeon at Premier Surgical Network in Egg Harbor Township and Cape May Court House, NJ. “But prolapsed hemorrhoids are something we treat routinely, and there are many options to help people feel comfortable again.”

To understand the symptoms, it helps to know what’s happening beneath the surface.

What Are Prolapsed Hemorrhoids and How Do They Develop?

Hemorrhoids are swollen veins inside the rectum or around the anus. Internal hemorrhoids form inside the rectum, where you can’t see them. When those internal hemorrhoids enlarge and push downward through the anal opening, they’re called prolapsed hemorrhoids.

Think of it like this: The veins inside the rectum act like cushions. Over time, and especially with repeated straining, those cushions can stretch and slip downward.

Common causes include:

  • Chronic constipation or diarrhea
  • Heavy lifting
  • Long periods of sitting
  • Pregnancy
  • Straining during bowel movements
  • Chronic coughing

Some people are more prone to developing them due to age, connective tissue changes, or ongoing digestive issues.

Understanding the Four Hemorrhoid Grades

Doctors classify prolapsed hemorrhoids into four grades based on how far they extend and whether they return inside the rectum.

  • Grade 1: Swollen internal hemorrhoids that do not slip beyond the rectum. You may notice bleeding but no visible bulge.
  • Grade 2: Hemorrhoids that protrude during a bowel movement but slide back inside on their own.
  • Grade 3: Hemorrhoids that need to be manually pushed back in.
  • Grade 4: Hemorrhoids that remain outside the body. These may become painful or develop complications such as clotting. Don’t confuse this with rectal prolapse, which is when part of the rectum drops outside of the anus.

As the grade increases, symptoms often become more noticeable and uncomfortable.

What Symptoms Indicate You Have Prolapsed Hemorrhoids?

Many people first notice:

  • A feeling of pressure or fullness
  • A soft lump during or after a bowel movement
  • Bright red blood on toilet paper
  • Itching or irritation

With more advanced prolapse, pain may worsen if the hemorrhoid becomes swollen or trapped outside the body. It can also become harder to clean yourself and wipe after using the bathroom, leading to more discomfort.

Bleeding that is heavy, persistent, or comes with severe pain should always be evaluated by a professional. Not all rectal bleeding is caused by hemorrhoids, so it’s important to be seen by an expert.

Hemorrhoidectomy vs Other Treatments: Which Is Right for You?

Mild cases often get better with dietary changes, fiber supplements, and over-the-counter medications. For more advanced grades, surgical treatment may provide lasting relief.

A traditional hemorrhoidectomy is the most common surgical approach. This involves surgical removal of the hemorrhoid and is often recommended for large Grade 3 or Grade 4 prolapse.

“The right procedure will depend on the grade of prolapse, symptom severity, and your overall health,” explains Dr. D’Angelo. “We always want to choose the least invasive option for our patients that sets them up for success.”

Recovery time varies by procedure, but most patients return to normal activity within a few weeks.
